General Data Protection Regulations
The Data Protection Act has been revised. From 25th May 2018 it will now be known as GDPR General Data Protection Regulations.
In line with changes to the Data Protection Act we have updated our policies
From the 25th May 2018 if you require someone to collect a prescription or any form of correspondence on your behalf we will require you to complete and sign a consent form giving permission for a named person to collect on your behalf.

The Practice takes our responsibilities associated with Data Protection seriously please find below a copy of our Privacy Notice which tells you how we handle and use your personal information. In addition to this should you require access to your information we would appreciate if you could complete a copy of the Practice's application form. Please ask at Reception.
