Request a Fit/Sick Note

If you are off work sick for seven days or less, you will need to self-certify. You will need a fit/sick note if you are off work sick for more than seven days. The seven days include days that you don't normally work. So when you work out how long you've been off sick, you should include weekends and bank holidays.

Self-certification Forms

Your employer can ask you to confirm that you've been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification.

How to request a Fit/Sick Note

1) Click on the following button to submit a request via the askFIRST app:

2) If you are unable to submit a request via askFIRST then please email us at
 SWLICB.COUNTRYPARKPRACTICE@NHS.NET with the following details:
 
- Your name and date of birth
- Whether you have already had a note about this
- Brief details about illness/medical problem
- When you would like the note to start and when you think you can return to work
- If possible, whether there is anything your employer can do anything to help you return sooner
 
We will be in touch with you when the fit/sick note is ready or if we have any further questions or futher action is required.
 
Fit/sick notes can be back-dated so please don't worry if your request is past the date you need the note to start from.
