Patient Triage Online – Starting 8 December 2025
From 8 December 2025, our practice will begin using Accurx Patient Triage to make it simpler and quicker for you to contact us. This new system replaces the AskFirst App (Sensely) and calling the practice to book an appointment. From this date, all requests should be submitted through Accurx Patient Triage using the secure online form. Just click the logos below for more information and to access the service.
Why we’re making this change:

You can request help for medical or administrative needs at any time, using your phone or computer.

Your request goes straight to our clinical team and will be prioritised based on what you need, not on call order.

If you can’t access the form online or don’t use digital devices, our reception team can help by completing the form with you over the phone.
We believe this change will improve access and ensure you are directed to the right person at the right time.
To help with the transition, we also encourage patients to download and create an account on the NHS App, which makes it easier to order prescriptions, view your appointment times and check medical information.

Q: How do I submit a request using Patient Triage?
A: Click the link or logo on our website, fill in the online form with your details and what you need, and submit. It takes only a few minutes and covers both medical and administrative help.
Q: What if I can’t access the form online or need help using it?
A: If you’re unable to use the online form, please call our reception team. We will support you over the phone and complete the form on your behalf to ensure you get the help you need.
Q: Will my request go directly to a doctor?
A: Yes. Every request is reviewed by our clinical team. A GP will decide the safest and most appropriate next step based on your clinical need.
Q: When does this change take effect and what happens to the AskFirst App?
A: The change takes effect on 8 December 2025. From that date, the AskFirst App (Sensely) will no longer be used. All requests must be submitted through Accurx Patient Triage.
Q: What should I do for emergency issues?
A: For emergencies, please call 999 or attend A&E immediately.
